Student Health ServicesImmunization Requirements

New full-time students must submit a completed Health History Form and provide evidence of having received the following:


  1. 2 doses of the Measles/Mumps/Rubella (MMR) vaccine after the first birthday

  2. 1 dose of the Meningitis vaccine for students under age 22

  1. 1 Tetanus/Diphtheria (Td) or Tetanus/Diphtheria/Pertussis (Tdap) vaccine booster within the last 10 years

  2. 3 doses of the Hepatitis B vaccine


Religious and medical exemptions can be requested through the Monarch Wellness Portal  

Complete your personal medical history online, enter immunization dates, and upload your immunization documents via Monarch Wellness Portal  



Full-time students are also required to complete the Tuberculosis (TB) Risk Assessment form.


  1. Students may also be required to show proof of a recent PPD Tuberculosis Skin test or IGRA blood test based on risk factors for TB.

  2. This form must be returned by US mail or uploaded through the Monarch Wellness Portal by August 1 for full-time students entering in the Fall or January 4th for full-time students entering in the Spring.

If this completed information is not received, a hold will block the student's registration for the second semester. If you prefer to submit the completed form by US mail, the form can be downloaded below. The mailing address is on the form.

Remember to keep a copy of your immunization forms for your records if you are mailing the form. Do not fax forms. They must be uploaded through the Monarch Wellness Portal or mailed.
