The University assumes no responsibility for illegitimate purchases from your account prior to your notifying the proper authorities that it is lost or stolen.
Lost your ID card? Act now!
If you lose your ID card, act right away to deactivate it! Contact the University Card Center at 757-683-3508 (M-F, 8AM-5PM), or the ODU Police Department at 757-683-4000 (after hours) to protect your card from misuse. Lost or stolen cards can also be deactivated online.
Replacement Cards
LOST OR STOLEN
Complete the Replacement Application and email it to cardcenter@odu.edu. You will be contacted with a date and time to come pick up your ID. Replacement ID cards are $15.00.
NAME CHANGE
Make sure your ODU ID reflects your new name! There is a $10.00 charge to replace your ODU ID Card due to a name change. Email cardcenter@odu.edu to learn more.
FOUND CARD
If you find your lost card, please visit the Card Center at 1120 Monarch Hall and present a driver's license or other form of photo ID to reactivate it.
Lost Card Meal Waiver
PLEASE NOTE
All customers wishing to use their meal plan or Monarch Plus account to purchase goods or services must present their University ID card for payment. In order to protect your account, there are no exceptions.
If you lose your meal card but still need to eat, please use the Lost Card Meal Waiver.
- This form will allow you to eat up to three meals at Cafe 1201.
- Any meals will be deducted from your account.
- False information or abuse of this policy will result in disciplinary action.
